BrightSpring Health Services is committed to the health and well-being of our employees and their families. Review the information below for an overview of BrightSpring’s comprehensive benefits program.

Healthcare Plan Options:
- Imagine360 is the plan administrator. There are several plan choices including two copay and two High Deductible Health Plan (HDHP) options.
- Plan Summaries:

- Use this tool to search for providers. The plans are open access meaning you can use providers of your choice.
- With Imagine360, plan participants are automatically eligible for free health programs to enhance your well being.
- Prescription administration is provided by Express Scripts.
- Use this tool to price medications.
- Employees working in CA, CO, OR and WA may be eligible for a Kaiser.

Dental and Vision Coverage:
- BrightSpring offers two dental plans administered by Delta Dental of Kentucky.
- The PPO Plus Plan provides coverage for major services including Orthodontia.
- The Preventative Plan covers basic services.
- BrightSpring Offers a choice of two vision plans administered by VSP:

Health Savings (HSA), Flexible Spending Accounts (FSA) and Other Reimbursement Programs:
- HSAs and FSAs are great companions to your medical enrollment. Review this chart for a comparison of the plans.
- BrightSpring offers a variety of reimbursement offerings from continuing education to adoption assistance.

Income Protection:
- BrightSpring offers both company provided and voluntary plans for life and disability.
- Other voluntary plans include Accident, Hospital and Critical Illness.

Retirement Plan:
- Employees are eligible to enroll after 90 days of continuous employment.

Part-time Benefits:
- Part-time employees are eligible for several plan options including a limited medical plan.
