Imagine360 Medical Plan Options
Plan Options:
Two Copay Plans are available.
Copay plans have fixed amounts for certain covered medical services like office visits and urgent care. Deductibles and co-insurance also apply.
Two High-Deductible Health Plan are also offered.
These plans have higher deductibles and lower monthly premiums than the Copay Plans. You are responsible for the full deductible for non-preventive services before the plan begins to pay.
