Imagine360 Medical Plan

Medical Plan Options
BrightSpring offers four Imagine360 medical plan options so you can choose what fits your care and budget. Review the Summary of Benefits and Coverage (SBC) and Plan Summaries for more information about each plan.
Copay Plans
Pay fixed copays for common services like office visits and urgent care. Deductible and coinsurance also apply for other services.
$3,500 Copay Plan
High-Deductible (HSA) Plans
Higher deductibles with lower monthly premiums. You pay the full deductible for non-preventive care before the plan begins to share costs.
$4,500 High-Deductible Plan with HSA
Other Regional Provider Partners
In select locations, access to additional provider networks are available. Members eligible for these networks will see the corresponding logo on their medical ID card. Review the Summary of Benefits and Coverage (SBC) below for additional information.

Provider Access & Network
BrightSpring’s medical plan, administered by Imagine360, is open access. This means you may use the providers and hospitals of your choice. While you’re not limited to a network, participants can access the MultiPlan Practitioner & Ancillary Network*. The network only includes providers and ancillary services. Hospitals are open access and do not require network affiliation.
*In select areas of NY, NJ, and CT, the network is MagnaCare, not MultiPlan. See extended network options for more information.

Follow these instructions to search the Multiplan network:

- Choose Practitioner & Ancillary
- Use the search tool to narrow your search.
Search Imagine360’s provider listing to find Provider Acceptance and Quality Ratings.
- Visit Imagine360’s search page
- Enter the code “OYZLP”
- Add your search criteria
Hospitals / Facilities
Hospitals are open access* and do not require network affiliation.
