Medical Plan Options
BrightSpring offers four Imagine360 medical plan options so you can choose what fits your care and budget. Review the Summary of Benefits and Coverage (SBC) and Plan Summary documents below for more information about each plan.
Copay Plans
Fixed copays apply to common services such as office visits and urgent care. A deductible and coinsurance apply for other services.

High Deductible Health Plans
Higher deductibles with lower monthly premiums. You pay the full deductible for non-preventive care before the plan begins to share costs.
$2,000 High Deductible Plan with HSA
$4,000 High Deductible Plan with HSA
Other Regional Provider Partners
In select locations, access to additional provider networks are available. Members eligible for these networks will see the corresponding logo on their medical ID card. Review the Summary of Benefits and Coverage (SBC) below for additional information.

Provider Access & Network
BrightSpring’s medical plan is open access. This means you may use the providers and hospitals of your choice. While you’re not limited to a network, participants can access the MultiPlan Practitioner & Ancillary Network. The network only includes providers and ancillary services. Hospitals are open access and do not require network affiliation.
In select areas, you may have access to extended networks. Please see below for more information.
